How to Replace a Failed HBA Configured in an Ibrix Environment

When replacing a failed HBA in an Ibrix configuration perform the following tasks:
Power off the failed server, replace the HBA, and then re-zone the switch with the new WWN. Reboot the server
and verify it can see all the storage with the lvscan and pvscan commands. It should display all the same lv's and
pv's as it's failover partner.
Remove the old HBA configuration from Ibrix:
Stop HBA monitoring:
ibrix_hba -m -U -h failed_server -p 20.00.00.e0.8b.1d.02.35 (example WWN)
Delete the old HBA address:
ibrix_hba -d -h failed_server -w 20.00.00.e0.8b.1d.02.35
Add the new HBA address:
ibrix_hba -a -h failed_server
Enable monitoring on new HBA address:
ibrix_hba -m -h failed_server -p <new WWN from previous command>
Once the configuration is complete and all the devices are accessible you can failback failed_server.
