Fill out this form completely and legibly. Return it with a 25-yards (approx.) piece of your line wrapped around a
business card or cardboard in such a manner that it can be unwound for testing. If you still have the receipt or spool,
please include it with the form. Upon receiving your line, we will send you a replacement spool. Please allow 2-3 weeks
for a reply. We appreciate your feedback, as it is very helpful to our ongoing research and development.
Please Note: We do not sell to dealers without a brick and mortar shop so we will only warranty products sold through
our regular retailers. We cannot warranty products that were purchased through on-line auction sites, i.e. E-bay, due to
the fact that we cannot trace the origin of the product.
