Subject to the terms set forth herein, I understand my purchase of a new qualifying Whirlpool® washer*

from an authorized Canadian Whirlpool® appliance retail dealer includes a 30-day Washer Vibration

Money-Back Guarantee. If I am not completely satisfied that my qualifying Whirlpool® Washer vibrates

less than my previous model, Whirlpool Canada LP will take it back within 30 days of the purchase date

and refund the full purchase price, including taxes, to me. Finance charges, any costs incurred in

connection with the removal of my previous washer and/or cost of delivery and installation of the

qualifying Whirlpool® washer are not covered by this Guarantee.

FOLLOW THESE STEPS TO SUBMIT A WHIRLPOOL® VIBRATION CONTROL MONEY-BACK

GUARANTEE CLAIM:

WITHIN 30 DAYS of the purchase of a qualifying Whirlpool® washer, complete the contact, qualifying

model and serial number information below and call the Whirlpool Canada Service Center at

1-800-807-6777 to initiate your claim. LATE SUBMISSIONS WILL NOT BE ACCEPTED.

A copy of your receipt showing dealer name and address, washer model and serial number and date of

purchase must accompany the claim. Retain a copy of all documents for your records. NOTE: All fields

must be completed to process your claim.
