Out-of-Service/Explant/Patient Death Form
Whenever a pulse generator is explanted, or if any of the leads or adapters are replaced or capped,
complete an Out-of-Service/Explant/Patient Death form and return it to Abbott Medical with the
explanted products. If possible, send along a printout of the programmed settings of the pulse
generator. For information on printing reports, see the appropriate reference manual.
