Reasons for Change:
• To replace existing tool due to end of tool life
• Improve tooling design to increase manufacturability of lens and reduce light trapped on uneven
surface of lens.
Effect of Change on Fit, Form, Function, Quality, or Reliability:
There is no change on material, device specification and product performance. The lens has comparable
performance as the existing lens base on reliability and characteristic test.
Effective Date of Change:
New changes will be implemented on 31 March 2014. Starting this date, shipment may contain either
existing parts or new parts until the existing materials are depleted.
