CLAIM PROCEDURE
To make a claim under this Warranty, the
original owner must, within 90 days of
actual or constructive notice of damage
covered by this Warranty, do the
following:
1. Prepare a letter which includes the
following information:
• A list of the number of pieces and
the size of each piece for which the
claim is made.
• Proof of Purchase of the product, as
shown on the original invoice.
• Proof of Warranty, as evidenced by
the original end-tag from the
product, or copy of the mark on
the product.
2. Mail the above information to:

Except as set forth herein, Warrantor
makes no other warranties. ALL IMPLIED
WARRANTIES INCLUDING, WITHOUT
LIMITATION, ANY IMPLIED WAR-
RANTIES OF MERCHANTABILITY OR
FITNESS FOR A PARTICULAR PURPOSE,
ARE DISCLAIMED. Some states do not
allow limitations on implied warranties,
so the above limitations may not apply to
you. IN NO CASE SHALL UFP BE
LIABLE FOR ANY INCIDENTAL OR
CONSEQUENTIAL DAMAGES. Some
states do not allow the exclusion of
limitation of incidental or consequential
damages, so the limitation may not apply
to you. This is a "limited warranty" within
the meaning of the Magnuson-Moss
Warranty - Federal Trade Commission
Improvement Act. This warranty gives
you specific legal rights and you may also
have other rights which may vary from
state to state.
No terms or conditions other than
those stated herein, and no agreement
or understanding, oral or written, in any
purporting to modify this warranty shall
be binding on UFP unless made in writing
and signed by its authorized representative.
This warranty will be valid only when all
outstanding bills and invoices related to
this Product are fully paid and settled and
will be void should there be any default of
payment within the terms and conditions.
This warranty applies only to the original
consumer of the product and is not
assignable or transferable and shall not be
effective unless and until signed by UFP
with signed copies exchanged between
UFP and the original consumer.
