Service Report: No fee or extra charge for this.
This is a report that is generated by the Repair Technician that has been assigned to repair a product that has been sent
back to the factory. The report is based on the state of the defective product compared to Product Specifications of a new
product. This report will consist of the following:
• The Part Number and Model Number of the product returned.
• The Serial Number (if applicable), Date Code and Warranty status.
• Description of problem the customer is experiencing, if provided.
• What the initial diagnosis was when the product was received compared to the New Product Specifications.
• Parts used to correct or repair the product.
• Product ECO’s (Engineering Change Orders) that were done to the product to get the product up to the latest rev.
Failure Analysis: Fee for this = $1500 per device (also applicable for in-warranty products).
This is a report that is generated by a Quality Engineer that has been assigned to analyze a product that has been sent back
to the factory to determine the root cause(s) of failure. At a minimum this report will consist of the following:
• The Part Number and Model Number of the product returned.
• The Serial Number (if applicable), Date Code and Warranty status.
• Customer name, address and contact name.
• Description of problem the customer is experiencing (background discussion).
• Performance analysis.
• Graphic data (where appropriate).
• Root failure cause(s) - Determination of initial event(s) leading to the direct cause of the failure.
• Recommend corrective action
• Verification of corrective action.
• Final or interim report.
What is required:
1. Purchase Order Number to cover $1500 per product.
2. Detailed problem of what the product is doing or not doing.
3. Application - Where and what type of facility the product is being used in.
4. End user Contact and Telephone information.
