RULES: Qualifying purchases made during May 1, 2015 and August 31, 2015 are eligible for rebate. A complete and accurate form must be submitted and received by Forney Industries no later than October 1, 2015.

Name on invoice must match name on rebate claim form. Only one rebate submission per invoice and/or receipt. Rebate cannot be combined with any other offers. Rebate checks will be paid to the individual person or

company named on the original invoice. Individuals may not redeem rebates on purchases made by or on behalf of the individual’s employer. Conrmation and status will be sent to email provided. Incomplete rebate forms

and/or missing proof of purchase will be returned. Only persons 18 or older may submit the rebate form and eligible to receive the rebate. Forney Industries interpretation of the rules is nal. Rebate offer will expire 30

days after August 31, 2015.
