SAMPLING DETAIL: Test samples were submitted to the laboratory directly by the client. No special sampling
conditions or sample preparation were observed by QAI.
DATE OF RECEIPT: Samples were received at QAI on June 29, 2015.
TESTING PERIOD: July 15, 2015- July 22, 2015.
AUTHORIZATION: Signed Work Order by David Malcolm, CEO of High Sierra Shower Heads.
TEST PROCEDURE: Test and evaluate the submitted samples to EPA WaterSense Specifications for
Showerheads.
TEST RESULTS: The samples meet the criteria of EPA WaterSense Specifications for Showerheads.
