outages in minutes. Section 1, “Introduction to Change Management,” describes
how to measure outages in minutes.)

The name of the person or group requesting the outage.

Applications affected by the outage.
Figure 2-1 shows an example of a planned outage request form.
Note. The maximum times on the following form are only examples. Times will vary from
installation to installation and can depend on a number of factors, including the size of the
configuration. You should modify this form based on your own needs and experience.
