Process Flow Diagram
After the change is successfully implemented and tested, the person or group responsible
for change control should maintain records of the changes that took place, including all
forms and signatures, and should document the results of the change.

Figure 2-2 is a high-level process flow diagram of how a change plan is implemented.
The diagram assumes that the following two conditions are met:
There is a single point of control in the form of a person or group with overall
authority to implement change.
All organizations necessary to support the change are committed to the plan.
