Cause. An ESM tape that was not requested is mounted because either:

The operator mounted the tape before the request was made.

The operator mounted the wrong tape.
Effect. The tape might be used to satisfy subsequent mount requests.
Recovery. Ensure that the correct tape is mounted and that a mount request is
issued.
