(such as channel reset), or a channel is defective.
Effect. The operating system disables the channel. All input/output (I/O) processes
using that channel switch to their configured backups. Channel-based I/O can no
longer be performed through this processor and channel; however, the processor and
any other channel remain up, and processes can run in the processor.
Recovery. Identify and replace the failing component (controller or channel). To
restore the channel to an operational state, reload the processor. (This action causes
processes running on the processor to stop.) Until the failing controller is replaced, this
error will recur. If the error continues to occur, see Recovery Action Requiring Vendor
Support on page 3-13.

The operating system reset a channel.
Unconditional Tokens
Cause. An excessive number of erroneous I/O or channel interrupts occurred; a failing
controller is connected to the specified channel. Either the controller does not identify
itself when polled, the controller does not respond to other forms of corrective action
(such as disabling the controller or switching the paths), or the channel is defective.
