Cause. Either a channel, a device controller, or the input/output bus cabling between
the channel and controller failed.
Effect. The operation in progress terminates abnormally.
Recovery. If the error continues to occur, see Recovery Action Requiring Vendor

event.
ZGIO-TKN-CONTROLLER Subject token that specifies the controller number for the
event.
ZGIO-TKN-UNIT Subject token that specifies the unit number for the event.
ZGIO-TKN-FECODE File-system error code returned when an invalid command
is sent to the device.
