Injuries involving this hazard pattern tend to be among the most serious of all playground injuries, and have the potential to be fatal, particularly when the injury is to the head.

Tools Required for Assembly: Two Adjustable Wrenches and Allen Wrenches. NOTE: It is strongly recommended that this equipment is assembled by two or more people to avoid possible injury. Ensure Carriage Bolts are inserted through the SQUARE holes on components that need to be assembled. Attach washer only to end of the Carriage Bolt. Use Allen Bolts or Hex Bolts inserted through the ROUND hole on components that need to be assembled.
1. Periodically inspect all parts. Check all nuts and bolts twice monthly during the usage season for tightness and tighten as required. It is particularly important that this procedure be followed at the beginning of each season. 2. Replace any worn and damaged part immediately. Check all coverings for bolts and sharp edges twice monthly during usage season to be certain they are in place. Replace when necessary. It is especially important to do this at the beginning of each new season. 3.
