Send in this completed order form together with the original register receipt or online receipt reflecting the purchase of any of the following NERF Blaster products: Vulcan EBF-25, LongStrike CS-6 Raider Rapid Fire CS-35, Recon CS-6, Stampede ECS, Deploy
CS-6, Nite-Finder EX-3, Barricade RV-10, Maverick REV-6. On the original register or online receipt, hand-print the UPC Code # (found at the bottom of the packaging) from the eligible product. Be sure to indicate on this form which qualifying blaster you have
purchased. Mail order form and original register receipt (NO RECEIPT COPIES, NO GIFT RECEIPTS) to the address listed above. Receipt must be dated 2/1/2011 through 3/31/2011. Allow up to 8-10 weeks following receipt of submission for verification and
postal mail delivery of your NERF darts. This offer cannot be combined with any other promotional offer, rebate, coupon or discount. Original store receipt may not be copied, reproduced, traded or sold. No submissions or receipts will be returned. Offer valid

STEP #3: Enclose your original register receipt (Gifts receipts and copies will not be accepted) indicating the purchase of a qualifying
NERF N-STRIKE BLASTER. On the original register receipt, write the UPC code # (This is the 10 numbers found below bar code
at the bottom of the packaging of the eligible product) for the qualifying product. We recommend that you print and keep a copy of
the register receipt for your own records.
