1. Use only on 110-120 volt, 60 Hertz.
2. Never disassemble or modify, any changes could void the user’s authority to
operate the device.
3. Not for use in totally enclosed fixtures.
4. Risk of electric shock-use in dry locations only.
5. This device is not intended for use with emergency exit fixtures or emergency exit
lights.
6. This product could produce interference when used near a equipment or other
critical navigation or communication equipment between 40-60KHz. If
interference should occur, move the lamp away from the appliance a distance of
three or plug the lamp or the appliance in to a different outlet.
7. To prevent burns do not touch the bulb during operation or immediately after
switching off.
8. Use only with 3-way dimmers.
9. Add weight of the device may cause instability of a freestanding portable.
10. Use only with portable lamps in which the distance from the bottom of the base to
the top of the lamp holder does not exceed three times minimum base width.
11. Use only with portable table lamps which are provide with lap shades.
12. Use indoors with open, surface mounted and track fixtures.
