SIGN-OFF FORM
• The equipment provider should follow the general Safety Standards
specified by the UN/DOT for the transport of packing Group III liquids.
• Anyone who will be using and/or maintaining the tank must read and
clearly understand ALL Safety, Usage and Maintenance information in
this manual
• Periodic reviews of SAFETY and OPERATION should be standard
practice for all your equipment
• A sign-off sheet is provided for your records, showing all personnel
working with the equipment
